Executive Order 13465, which amended Executive Order 12989, required contractors with covered federal contracts to use an electronic employment verification system (E-Verify) to verify the employment eligibility of employees performing work on a federal contract. Under the Right to Privacy in the Workplace Act (820 ILCS 55), the Illinois Department of Labor also requires employers conducting business in Illinois to complete an attestation using this form prescribed by the department.
